About Nanton Community Health Centre

Nanton Community Health Centre is a full service family practice, seeing patients over their entire life span.  We use a team approach to health care which means you will encounter physicians, nurse practitioners and other health professionals within our clinic such as Lab services, Public Health and Home care.  An inter professional team will be involved in providing your care in collaboration with Alberta Health Services, Alberta Health and Calgary Rural Primary Care Network.

Alberta Health’s Clinical Alternative Relationship Plan (ARP) Program compensates your Physicians. Your Physicians receive an annualized compensation - other than fee-for-service, to provide defined clinical services.  
Your Nanton Community Health Centre Clinical ARP was established to enhance Physician recruitment and retention, team-based approaches to service delivery, access to services, patient satisfaction, and value for money. The physicians lease space from AHS to provide these services and are independent providers not employees of AHS

AHS (Alberta Health Services)
Nanton Community Health Centre is an AHS Program.  AHS leases the building from the owner (Owner: Town of Nanton, MD’s of Willow Creek and Ranchland).
AHS provides Clerical Staff and Nurse Practitioners to support the Physicians providing your primary care.
AHS also provides other services out of Nanton Community Health Centre including

• Community Health (Home Care)
• Public Health
• Adult Day Support
• Mental Health
• Lab Services (Alberta Precision Labs), Collection site
• Social Work (PCN funded)

AHS provides and maintains the medical equipment and telephone/IT equipment, as well as provides all medical and office supplies.

PCN (Primary Care Network)
The Calgary Rural Primary Care Network works with family doctors to develop your medical home.  A Medical Home is your doctor supported by a multi-disciplinary health-care team helping to provide a continuum of comprehensive care based on best practices in medicine.   
The PCN connects you to the specialists you need, supports long-term relationships with your medical champions, and helps you take your healthy future into your own hands. It's about building healthy communities and a healthier YOU!  
The PCN supports and provides many enhancements that the Nanton team identify would be a beneficial service to the local community.
