Home Care
What is Home Care?
Home Care assists clients of all ages to stay in their homes with safety, dignity and quality of life. All services are based on assessed need and may be provided by a Registered Nurse, Licensed Practical Nurse, Health Care Aide, Social Worker, or Therapy Assistant. Care is provided either at the Nanton Community Health Center or in the client’s home.

Services Provided:
- Case Management
- Intravenous Therapy
- Wound care
- End of Life Care (including pain & symptom management and referral to hospice),
- Referral to Long Term Care or Supportive Living Facilities
- Social Work – grief support, assistance accessing financial supports, etc
- Assessment for supplies and equipment from Alberta Aides to Daily Living Program
- Health Care Aides services – ie bathing assistance, respite care, medication assistance
- Adult Day Program – provides socialization, recreation and health monitoring to people who are isolated at home.

Public Health
The goal of our Public Health team is to work with Nanton and area community members and groups to reduce the incidence of illness, injury and communicable diseases through Health Promotion and Prevention strategies.

Lab
Hours of Operation:
Monday- Thursday 8:30 am- 12 pm and 1 pm- 4:15 pm Friday 8:30 am - 12 pm The last blood draws are completed 15 minutes prior to closing. Our Lab services are provided by Alberta Precision Laboratory (company with AHS) services with Medical Lab Assistants. Our lab provides collection of blood and urine specimens for tests ordered by your doctor Nurse Practitioner or other healthcare provider. This service provides a comprehensive range of routine and specialized lab testing, services include:

Mental Health
Provides programs and services for people and their families experiencing mental health issues. Examples of the programs and services Mental Health can assist with include: abuse, violence, grief, stress, aging, relationships, anxiety, depression, suicidal thoughts, mental illness that doesn’t get better, and severe mental illness.

Social Work
- Provides caregiver support.
- Assists accessing financial social support services.
- Assists patients and their families with transition and placement planning.
- Assists patients by brokering community resources.
Diabetic Clinic
Team consisting of a specially trained RN and Dietitian visit monthly to visit with referred patients for support therapy, education, maintenance and monitoring.
Travel Clinic Options
Clients have a few options when seeking travel vaccines, There is a fee for all travel clinics.

- Physicians may also provide travel vaccine to their Patients but this is up to the physician. They only provide the vaccine not the travel consult, this is determined by the individual physician’s office.
- Pharmacies also provide travel vaccine ie shoppers in Okotoks, you could check with the local Nanton pharmacy-I’m not sure if they do travel vaccines or not. Pharmacy dependent on if they provide vaccine only or vaccine and consult.
- Private travel clinics are also available in Calgary-you can just google Calgary Travel clinics and they will come up.
